We send a short health-history form by email. Fill it before you arrive.

First visit

Cleaning, exam, x-rays as needed, plus a personalized treatment plan.

A short list. None of these are deal-breakers, just helpful for a faster first visit.

  • Health card
  • Insurance card (we direct bill many insurers)
  • List of current medications
  • Name of your previous dentist (so we can request records)

Quick answers.

Do you accept new patients? +

Yes, always. New patients are welcome at both Dr. Kawi and Dr. Dania Hadi.

Do you direct bill insurance? +

Yes. We submit electronically to most Canadian insurers. We will let you know about any out-of-pocket portion before treatment.

I am nervous about dental work. Help. +

Sedation dentistry is available for nervous patients. We will talk through options at the consult.

Is there free parking? +

Yes. Free on-site parking in front of the building at 1103 Adelaide St N.
